First Time Appointments

Please arrive 15 minutes prior to your scheduled appointment with a photo ID and insurance card. Our staff will request that you fill out a couple of forms upon your first visit including the ‘Welcome Form,’ attached below. To save time, feel free to print and fill out the forms before you arrive, otherwise we will provide them for you.

Please note that we reserve the right to charge a fee for any scheduled appointments that are either missed or canceled within less than 24-hour notice.

Cancellation Policy

Insurance

At the Center for Glaucoma & Cataract Care, we are medicare partners and accept most major insurance plans. If you have any questions regarding your specific plan we recommend contacting your insurance company directly.

  • Feel free to call and ask about your particular insurance plan

  • Services not covered by insurance are the patient’s responsibility

  • At this time, we do not participate with any vision plans

We are available to make appointments and answer questions during our regular business hours from 8:00am to 5:30pm, Monday through Thursday. We are open half days on Fridays from 8:00am to 12:00pm. If you have an eye emergency after business hours, please reach out to your urgent care center.
